Physiotherapist – Community Home Care
Stream Healthcare are seeking a passionate Community Physiotherapist to deliver high-quality, in-home care to clients of all ages across South Brisbane and Logan and the surrounds. Caseloads are funded through Home Care Packages, NDIS, or privately managed.
Key Responsibilities:
- Conduct comprehensive in-home assessments and develop personalised treatment plans
- Set SMART goals and educate clients and families for long-term health outcomes
- Collaborate with multidisciplinary teams
- Maintain clear, timely documentation and reports
- See up to 8 clients per day (initial assessments and follow-ups)
- Participate in team meetings and professional development
- Full Drivers Licence
- Own Vehicle fully insured and road worthy
- Bachelor/Masters or Doctorate of Physiotherapy
- Current Police Check Clearance
- Working with Children Check
- Full or Limited Registration with Australian Heath Practitioner
- Regulatory Authority (AHPRA)
